Diabetes Mentor

“We have set up a Task & Finish Sub-Group of the Self-Care & Patient Education Project Group to make recommendations for the expansion of Diabetes Awareness Champions and Mentors in to all eight CCG areas in NWL. We would very much like to have an Ealing Champion member of this Group.

The Diabetes Awareness Champions deliver key messages about diabetes like:

  • What are the symptoms?
  • How do you get tested?
  • What can you do to prevent or delay the onset of diabetes (diet and exercise)?
  • Where can you get more information?

They target particularly BME communities and areas of deprivation. They also provide diabetes information leaflets and can be found at health fairs, other events and at religious and community groups. Diabetes UK has trained a group of champions in Ealing. Are you aware of this? They may be linked to Public Health.

Diabetes Mentors are based at GP surgeries which have diabetes clinics. They have one on one discussion with patients who have issues they need to talk about. The purpose of Task and Finish Group 4 is to make recommendations for the further development of champions and mentors across all eight CCG areas in North West London.

Further information about the Triborough Champions and Mentors can be found on the Professionals tab of www.knowdiabetes.org.uk”

Please note that the first meeting is taking place on Friday, 1 September, 2.00 to 4.30 pm on Friday 1st September at North West London NHS offices at 15 Marylebone Road, London NW1 5JD in room 2.1.
